Is there a form for an employee to sign that would require them to pay back educational fees"?

The company pays for special training, the employee would be required to sign a contract that would have them pay back a certain portion of the educational fees should they leave the job before a certain time period. I would like a template for that emplyee to sign

There are probably templates that you could use or purchase on the net but the safest way is to have a lawyer draw one up. If you only want to use it internally and understand that it may not be usable in court, you can draw up an easy contract of your own and save the money of a lawyer.

You appear to be the employer. Seek an attorney's advise or talk to someone like a registrar, Anything you do or require of the employee needs to be binding within the parameters you want set.

Your best bet is to consult a lawyer.
