Employer doesn't have handbook; FMLA rights?

I have several questions about FMLA (Georgia) I'm hoping someone can point me in the right direction. My company doesn't have a handbook (kinda makes up rules as they go) but meets the qualifications for FMLA (over 50 employees within 75 miles, etc) I meet the qualifications as well.

I read an article on Baby Center that I need a FLMA application to my employer...where can I get one?

I plan on taking 8-10 weeks leave when the baby is born but don't know if I will need bed rest before that. Can they deny my request if I have to take off earlier than the date I put on the application? If so, can they deny my request if I put an earlier date but don't leave until after that?

Am I required to keep in contact with my employer while I am out on leave? I know they will call me all day long with questions, and I simply don't want to deal with work while I have a newborn at home.

What happens if I need to take time off when they baby is say, 8 months old?

The person who does HR doesn't know anything about FMLA or any policies that are in place. She has been here 25 years. :(

Everything you need is at the website:

Go talk to your Human Resources representative at work. They should be able to answer your questions...One piece of advise. Any thing you may need as mentioned in your question---needs to be in writing and signed by someone in the HR office. Get a copy of the document (it serves as a piece of evidence in the event you are short changed in the agreement later on). Be sure to ask about information concerning maternity leave (all aspects-pre and post).
